COACHING AGREEMENT

I hereby voluntarily request and consent to receive Coaching services from Sarah Yoga & Wellness. I understand and acknowledge that no guarantees have been made to me as to the effect of such services.

I further understand and acknowledge that in no way are these services meant to be construed by me as the diagnosis or treatment of disease, but rather an aid to balancing my energy and to possibly improving my general wellness, and promote healing on a physical, mental, emotional and Spiritual level; and also assist in relaxation and stress reduction.

I understand that Sarah Yoga & Wellness, upholds the highest standards of care and professionalism.  I

understand that Coaching can assist in transforming attitudes and energies to create a more positive life situation, and improve my general health and well being.

I understand that Coaching is not a substitute for mental health treatment or medications, and it is recommended that I concurrently work with my doctor or mental health professional for any condition I may have.  I am advised that if I am sick, I should consult my doctor, and or mental health professional.  I am aware that my the practitioner does not diagnose illness or disease, and does not prescribe medication.

I understand that all of the advice rendered to me by Sarah Ceo or any other teachers is of a non-medical nature and any actions I take, personally or professionally, and the consequences that flow from these actions, are solely within the domain of my responsibility. Neither Sarah Ceo, nor Sarah Yoga & Wellness LLC assumes any liability.

MEDIA RELEASE WAIVER

I grant permission to Sarah Yoga & Wellness to record the classes and coaching sessions I participate in and use my image (photographs, video, and/or audio) for use in training and promotional purposes.

I understand that Coaching 1-1 sessions will be conducted in 60-minute Zoom video calls and broadcasted LIVE on the internet.

I grant permission to Sarah Yoga & Wellness and it's affiliates, to use my image (photographs, video, and/or audio) for use in media publications. 

I hereby waive any right to inspect or approve the finished photographs or electronic matter that may be used in conjunction with them now or in the future, where that use is known to me or unknown.  I waive any right to royalties or other compensation arising from or related to the use of the image.
